Position Overview

Primary Care Careers is seeking a dedicated pharmacy technician to join our team on a part-time basis, working 20 hours per week.

Key Responsibilities

  • Collaborate with clinical pharmacists and healthcare professionals in line with established prescribing protocols.
  • Assist in conducting medication reviews and ensure accurate medicines reconciliation following patient transitions from hospital care.
  • Support the clinical pharmacist in organizing necessary monitoring tests for Structured Medication Reviews (SMR).
  • Contribute to the achievement of targets outlined in the PCN DES and IIF contract.
  • Facilitate effective medication use through shared decision-making discussions with patients.
  • Drive improvements to enhance safe and cost-effective prescribing practices, ultimately improving patient care quality.
  • Guide patients in optimizing their medication regimens by adhering to approved protocols.
  • Enhance repeat prescribing processes within the PCN, promoting repeat dispensing and online ordering while minimizing clinical risks and reducing medication waste.
  • Assist in meeting national standards, implementing NICE guidelines, and utilizing medicines optimization initiatives.
  • Engage in patient support roles to ensure effective medication use through collaborative discussions.

Our Primary Care Network consists of four GP practices working together to deliver a positive experience for our patients.

Qualifications

Essential Requirements

  • Professional registration with GPhC.
  • BTEC/NCQ level 3 or equivalent in Pharmaceutical Sciences.

Experience

Essential Requirements

  • A minimum of 2 years of experience as a pharmacy technician in a hospital, community, or primary care environment.
  • Proficient in patient interactions both in-person and through digital channels.
  • Experience working within a diverse team across multiple disciplines.
  • Exceptional verbal and written communication skills, with the ability to tailor communication style to various audiences.
  • Willingness to enroll in or pursue an approved training pathway (PCPEP/CPPE).
  • Strong computer skills, particularly with MS Office applications.
  • Familiarity with System One is preferred.
  • Thorough understanding of data protection and confidentiality standards.
  • Attention to detail with the ability to identify errors promptly.
  • Self-motivated and proactive in approach.
