We are seeking a highly skilled and compassionate Community Palliative Care Specialist Nurse to join our team at the Central London Community Healthcare NHS Trust – Hertfordshire Division.

About the Role

This is an exciting opportunity to work as part of our integrated Cancer, Non-Cancer, and Palliative Care team, delivering exceptional care to patients with palliative and end-of-life needs.

Key Responsibilities
  • Provide highly specialist palliative care advice and support to other generalist nurses, care agencies, and health professionals.
  • Develop and implement individualized care plans, utilizing expert skills in communication, clinical judgement, and management.
  • Collaborate with a multi-professional team to ensure high-quality patient care, working across organizational boundaries.
  • Contribute to the development of the team, sharing knowledge and expertise to enhance palliative care services.
Requirements
  • Registered Nurse with current UK NMC registration.
  • Experience working at a senior level in palliative or end-of-life care, with a strong understanding of current political and policy context in Primary health Care.
  • Ability to use influencing, negotiating, and counselling skills in a highly emotive atmosphere.
  • Hold a full, valid UK driving licence and have access to a car for business purposes.
