Position Overview

The Clinical Nurse Specialist is tasked with delivering high-quality and effective nursing care to the entire patient population at Meadowgreen Health Centre. This role involves close collaboration with the Lead Practice Nurse and General Practitioners to achieve the healthcare objectives of the practice. The primary focus is on providing evidence-based care for patients with chronic conditions and ensuring appropriate nursing interventions for all individuals.

Key Responsibilities

Clinical Accountability

  • Report to the Lead Practice Nurse for daily operational matters.
  • Accountable to the Partners for clinical and professional standards.
  • Coordinate with the Practice Manager regarding employment-related issues.

Core Functions

  • Advise practice partners on nursing-related matters and contribute to the overall development of the healthcare facility.
  • Collaborate with community nursing teams, including district nurses and health visitors, to coordinate patient care.
  • Assist in the development and execution of recall systems for patients registered with chronic diseases.
  • Evaluate patients with complex health needs and formulate care plans, including specialist interventions.
  • Design, implement, and assess health improvement programs tailored to individual and group needs.
  • Analyze and report on health data to inform practice population needs.
  • Contribute to the formulation and monitoring of policies and guidelines within the nursing practice.
  • Ensure efficient use of healthcare resources.
  • Support the professional growth of students and staff.
  • Promote a safe and secure environment for colleagues and patients.

Clinical Practice Responsibilities

  • Assess, plan, and evaluate treatment programs that enhance health and well-being.
  • Manage treatment plans for patients with known chronic conditions.
  • Identify and manage patients at risk of developing chronic illnesses.
  • Collaborate with healthcare professionals to monitor and treat chronic conditions, utilizing evidence-based practices.
  • Review and adjust medications based on therapeutic effectiveness and patient needs.
  • Support patients in adhering to prescribed treatment regimens.
  • Provide guidance on medication, including potential side effects and interactions.
  • Prioritize health issues and respond effectively in urgent situations.
  • Encourage health promotion strategies and self-care among patients.
  • Assess and refer patients with mental health concerns as necessary.
  • Deliver evidence-based care for patients with ear-related conditions.
  • Address the needs of patients requiring wound care.

  • Utilize effective communication techniques to ensure patients are informed and consent to treatments.
  • Support patients receiving difficult news with sensitivity.

Qualifications and Experience

Essential Qualifications

  • Registered Nurse qualification.
  • Experience working within a nursing team.
  • Experience with vulnerable patient populations.

Desirable Qualifications

  • Postgraduate diploma or degree in Chronic Disease Management.
  • Nurse prescriber qualification.
