**Job Title:** Social Worker

**Job Summary:**

We are seeking a skilled and compassionate Social Worker to join our team in Powys. As a Social Worker, you will play a vital role in promoting the wellbeing and independence of individuals and families in our community.

**Key Responsibilities:**

* Conduct assessments of need under the Social Services and Wellbeing Act 2014
* Develop and implement robust risk assessments to ensure the safety and well-being of service users
* Collaborate with health colleagues and statutory agencies to provide high-quality services
* Promote community inclusion and support service users to achieve their goals
* Work in a strengths-based, person-centered approach to support service users

**Requirements:**

* Active listening and empathy skills
* Excellent verbal and written communication skills
* Ability to work in a fast-paced environment and prioritize tasks effectively
* Enhanced DBS Check
