About the Role

We are seeking a highly skilled and dedicated Social Worker to join our Integrated Adult Independence Team. As a key member of our team, you will play a vital role in promoting the independence, well-being, and quality of life for adults in our community.

Key Responsibilities
  • Conduct thorough assessments under the Care Act 2014 and other relevant legislation to ensure the needs of vulnerable adults are met.
  • Develop and implement tailored support plans to address the individual circumstances of service users.
  • Work closely with service users, their carers, and multi-disciplinary partners to ensure a strengths-based approach to service delivery.
  • Manage a caseload and conduct robust assessments in partnership with service users and their carers.
Requirements
  • 2 years' experience post-qualification in a relevant field.
  • Ability to work with safeguarding procedures and protocols.
