Job Overview

We are seeking a dedicated Multidisciplinary Team Coordinator and Patient Pathway Facilitator who aligns with our mission to deliver exceptional care and service to our patients.

The successful candidate will be accountable for ensuring that all patients with suspected or confirmed cancer are effectively monitored throughout their designated patient pathway from the initial point of contact with the Trust. This involves collaboration with both clinical and administrative teams to guarantee that each phase of their journey is meticulously organized and scheduled, thereby meeting Cancer Wait Times objectives.

You will play a crucial role in streamlining the patient experience by establishing coordinated administrative processes. This includes managing a Patient Tracking List (PTL) and facilitating Multidisciplinary Team (MDT) meetings.

Key Responsibilities

The MDT/Patient Pathway Coordinator will oversee patients referred by their General Practitioners with a suspicion of cancer.

A solid understanding of NHS and Trust systems is required, along with proven administrative skills, including proficiency in word processing and spreadsheet applications, as well as familiarity with medical terminology and the ability to perform live data entry during MDT meetings.

You should be capable of working in a calm, logical, and methodical manner while managing multiple tasks in a dynamic environment and engaging with individuals at all levels, both internally and externally.

Ensure that pertinent clinical information is available for discussion during meetings, including patient notes, investigations, and test results, which may involve liaising with external hospitals for necessary data.

Attend weekly MDT meetings and maintain accurate records of attendance for the multidisciplinary team in accordance with Peer Review standards.
