Position Overview

The Care Home Liaison (CHL) team operates as a dedicated service collaborating with staff in various care facilities throughout Lanarkshire, focusing on residents facing mental health challenges. As a Registered Nurse, you will be responsible for managing and providing care to a designated group of clients, ensuring the implementation of the Nursing Process and case management principles to maintain high-quality patient care while partnering with other healthcare professionals.

Key Responsibilities

  • Hold a valid NMC Registration as a Registered Nurse.
  • Possess a degree-level education or be working towards it.
  • Demonstrate postgraduate expertise in your specialty, showcasing the necessary breadth of knowledge to lead effectively.
  • Exhibit comprehensive understanding of clinical guidelines and standards relevant to your area of practice.
  • Utilize specialist knowledge and experience to provide leadership within your specialty.
  • Exhibit strong leadership skills for the daily management of service delivery.
  • Engage in continuous professional development, including relevant courses and qualifications.
  • Demonstrate excellent teamwork and leadership abilities, with a capacity to motivate and work independently.
  • Possess strong communication and interpersonal skills.
  • Manage time effectively to meet service demands.
  • Have a working knowledge of basic IT systems.
  • Be flexible in working hours to accommodate service needs.
  • Hold a full, valid UK/EU/EEA driving license for community-based roles.

Preferred Qualifications

  • Training in geriatric psychiatric care.
  • Evidence of ongoing professional development in your specialty.
  • Experience in community mental health services.

Contract Details

This is a permanent, full-time position requiring 37 hours per week.
