Key Responsibilities- Develop, implement, and embed health promotion and wellbeing programmes
- Manage patients presenting with a range of acute and chronic medical conditions
- Implement and evaluate individualised treatment plans for chronic disease patients
- Identify, manage, and support patients at risk of developing long-term conditions
- Provide advanced, specialist nursing care to patients as required
- Provide wound care, including ulcer and Doppler assessments
- Provide specialist clinics, such as ear syringing and contraception
- Undertake the collection of pathological specimens
- Provide travel medicine services
- Request pathology services as necessary
- Process and interpret pathology and other test results
- Provide chronic disease clinics and refer patients to secondary/specialist care as required
- Maintain accurate clinical records
- Ensure SNOMED CT codes are used effectively
- Maintain chronic disease registers
- Develop, implement, and embed well woman, well man, and travel clinics
- Develop, implement, and embed an effective call/recall system
- Be responsible for the implementation of an effective immunisation programme
- Chaperone patients where necessary
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Recognise, assess, and refer patients presenting with mental health needs
- Implement vaccination programmes for adults and children
- Be an extended and supplementary prescriber
- Support patients in the use of their prescribed medicines
- Contribute to practice targets and comply with local and regional guidance
- Liaise with external services/agencies to ensure patient support
- Delegate clinical responsibilities appropriately
- Support the clinical team with safeguarding matters
- Understand practice and local policies for substance abuse and addictive behaviour
- Deliver opportunistic health promotion
- Provide supervision and support to other staff members
- Attend clinical, MDT, and practice meetings
- Undertake any tasks consistent with the level of the post and the scope of the role
- Deliver Routine ANP appointments in line with the criteria set out by the senior partner
- Deliver Minor Operations in line with service specification
- Deliver Cryotherapy in line with the service specification
- EMD clinics
- 6 week baby checks
- Deliver joint injections in line with the service specification
- Request imaging tests in line with IRMER specification
- Act as the audit lead, effectively utilising the audit cycle
- Act as the IPC lead for the practice, ensuring compliance, undertaking audit as necessary
- Monitor and ensure the safe storage, rotation, and disposal of medicines
- Participate in local initiatives to enhance service delivery and patient care
- Support and participate in shared learning within the organisation
- Develop an area of specialist interest, taking the lead within the organisation
- Continually review clinical practices, responding to national policies and initiatives
- Participate in the review of significant and near-miss events
- Drive the development of nursing services within the organisation
- Develop practice administrative and clinical protocols
