We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team at Brook Medical Centre.

Job Summary

The successful candidate will be responsible for providing high-quality patient care, managing complex medical conditions, and leading clinical teams. As an Advanced Nurse Practitioner, you will have the opportunity to develop and implement health promotion and wellbeing programmes, manage patients with acute and chronic medical conditions, and provide advanced nursing care.

Key Responsibilities
  1. Develop, implement, and embed health promotion and wellbeing programmes
  2. Manage patients presenting with a range of acute and chronic medical conditions
  3. Implement and evaluate individualised treatment plans for chronic disease patients
  4. Identify, manage, and support patients at risk of developing long-term conditions
  5. Provide advanced, specialist nursing care to patients as required
  6. Provide wound care, including ulcer and Doppler assessments
  7. Provide specialist clinics, such as ear syringing and contraception
  8. Undertake the collection of pathological specimens
  9. Provide travel medicine services
  10. Request pathology services as necessary
  11. Process and interpret pathology and other test results
  12. Provide chronic disease clinics and refer patients to secondary/specialist care as required
  13. Maintain accurate clinical records
  14. Ensure SNOMED CT codes are used effectively
  15. Maintain chronic disease registers
  16. Develop, implement, and embed well woman, well man, and travel clinics
  17. Develop, implement, and embed an effective call/recall system
  18. Be responsible for the implementation of an effective immunisation programme
  19. Chaperone patients where necessary
  20. Prioritise health issues and intervene appropriately
  21. Support the team in dealing with clinical emergencies
  22. Recognise, assess, and refer patients presenting with mental health needs
  23. Implement vaccination programmes for adults and children
  24. Be an extended and supplementary prescriber
  25. Support patients in the use of their prescribed medicines
  26. Contribute to practice targets and comply with local and regional guidance
  27. Liaise with external services/agencies to ensure patient support
  28. Delegate clinical responsibilities appropriately
  29. Support the clinical team with safeguarding matters
  30. Understand practice and local policies for substance abuse and addictive behaviour
  31. Deliver opportunistic health promotion
  32. Provide supervision and support to other staff members
  33. Attend clinical, MDT, and practice meetings
  34. Undertake any tasks consistent with the level of the post and the scope of the role
Additional Responsibilities
  1. Deliver Routine ANP appointments in line with the criteria set out by the senior partner
  2. Deliver Minor Operations in line with service specification
  3. Deliver Cryotherapy in line with the service specification
  4. EMD clinics
  5. 6 week baby checks
  6. Deliver joint injections in line with the service specification
  7. Request imaging tests in line with IRMER specification
Leadership and Quality Improvement
  1. Act as the audit lead, effectively utilising the audit cycle
  2. Act as the IPC lead for the practice, ensuring compliance, undertaking audit as necessary
  3. Monitor and ensure the safe storage, rotation, and disposal of medicines
  4. Participate in local initiatives to enhance service delivery and patient care
  5. Support and participate in shared learning within the organisation
  6. Develop an area of specialist interest, taking the lead within the organisation
  7. Continually review clinical practices, responding to national policies and initiatives
  8. Participate in the review of significant and near-miss events
  9. Drive the development of nursing services within the organisation
  10. Develop practice administrative and clinical protocols
