About the Role

We are seeking a skilled and enthusiastic Community Mental Health Nurse to join our team as a Care Coordinator in Bridgend South CMHT. As a key member of our multidisciplinary team, you will work closely with patients, families, and other healthcare professionals to provide high-quality care and support.

Key Responsibilities

  • Assess and develop care plans for patients with varying mental health needs and diagnoses.
  • Work collaboratively with the multidisciplinary team to provide comprehensive care and support.
  • Conduct regular reviews and assessments to ensure patients receive the best possible care.
  • Develop and maintain effective relationships with patients, families, and other healthcare professionals.
  • Participate in joint working and contribute to the development of care plans.

Requirements

  • Must be a qualified RMN with experience in mental health nursing.
  • Ability to communicate effectively and clearly.
  • Experience in the use of risk assessments.
  • Ability to problem solve and work autonomously.
