Nurse Practitioner

We are seeking a skilled and dedicated individual to join our team as a District Nurse. As a key member of our Community Nursing Service, you will play a vital role in providing high-quality patient care and support to residents in Stockport.
Key Responsibilities- Work collaboratively with Community Nurses and Adult Social Care staff to deliver a first-class service to patients in their homes and residential settings.
- Provide clinical expertise and support to patients with complex needs, working closely with carers and other healthcare professionals.
- Contribute to the development and implementation of care plans, ensuring patient-centered and person-centered approaches.
- Participate in multidisciplinary teams to share knowledge, best practices, and expertise, promoting a culture of continuous learning and improvement.
- Engage with patients, families, and carers to understand their needs, preferences, and concerns, promoting open communication and empathy.
- Current registration with the Nursing and Midwifery Council (NMC).
- Grade C (or equivalent) in GCSE English.
- Desirable: Diploma in Nursing Practice or degree, Teaching and Assessing Certificate, or evidence of health-related study at level 2/3.
