Job Title: Band 5 Student District Nurse

We are seeking a highly motivated and enthusiastic Band 5 Student District Nurse to join our team in the borough of Kingston. As a key member of our community nursing team, you will be responsible for providing skilled nursing care to patients in their own homes and other community settings.

Main Responsibilities:
  • To assess, plan, implement, and evaluate programmes of care for service users and their carers.
  • To work collaboratively with the wider multi-disciplinary team and other health and social care practitioners to provide the best care and quality outcomes for patients and service users.
  • To participate in health promotion activities and contribute to the development of health and social care services in the community.
  • To work effectively with colleagues in the Integrated Long Term or intermediate short-term Care teams, members of the Primary Health Care team, and the hospital setting.
Requirements:
  • Registered General Nurse with a minimum of 2 years community experience.
  • Relevant post-registration study at diploma/degree level.
  • ENB 998 or equivalent Mentor or Practice Assessor course.
  • Evidence of continuing professional development.
  • District Nurse Specialist Practitioner qualification or willingness to work towards.
  • Independent Prescriber or willingness to work towards non-medical Prescribing.
  • Relevant courses in Long Term Conditions Management.
  • Clinical Decision Making qualification or willingness to work towards.
Skills and Abilities:
  • Excellent communication and interpersonal skills.
  • Ability to work without supervision while maintaining strong teamwork.
  • Teaching skills.
  • IT skills including Excel, Word, and database.
  • Ability to utilise research in clinical practice.
Personal Qualities:
  • Highly self-motivated.
  • Flexible to deal with situations that may lead to adjustments within teams' workload.
