About the Role
Leeds Community Healthcare NHS Trust is seeking a skilled and compassionate Nurse to join our Community Care Bed (CCB) team. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to patients in their homes and local health centers.
Main Responsibilities
- Provide expert nursing care to patients in their homes and local health centers, working independently and as part of a wider team.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and effective communication.
- Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed.
- Participate in quality improvement initiatives to enhance patient outcomes and service delivery.
Requirements
- Registered Nurse with a valid UK nursing license.
- Experience working in community health or adult care settings.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a flexible approach to shift patterns and workloads.
- Commitment to delivering high-quality patient care and contributing to a culture of excellence within the organization.
