About the Role

We are seeking a highly skilled and experienced Senior Discharge Coordinator to join our team at Dorset HealthCare University NHS Foundation Trust. As a key member of our discharge planning team, you will play a vital role in ensuring the smooth transition of patients from hospital to home or other care settings.

Your primary responsibility will be to coordinate the discharge process, working closely with patients, carers, and healthcare professionals to identify and address any barriers to discharge. You will also be responsible for ensuring that all necessary documentation is completed accurately and in a timely manner.

Main Responsibilities
  • Coordinate the discharge process for patients, including identifying and addressing any barriers to discharge
  • Work closely with patients, carers, and healthcare professionals to ensure a smooth transition from hospital to home or other care settings
  • Ensure that all necessary documentation is completed accurately and in a timely manner
  • Develop and maintain effective relationships with patients, carers, and healthcare professionals to ensure a high-quality patient experience
  • Contribute to the development and implementation of discharge planning policies and procedures
Requirements
  • Experience of working in a healthcare setting, preferably in a discharge planning or patient flow role
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals
  • Ability to work autonomously and as part of a team, with a high level of organization and time management skills
  • Knowledge of relevant legislation and policies, including the Mental Capacity Act and the Care Act
  • Ability to maintain confidentiality and handle sensitive information with discretion
