Job Summary
We are seeking a highly motivated and experienced Community Integrated Nurse to join our East Dorset ICRT Team. As a key member of our team, you will play a vital role in delivering an Admission Avoidance, Discharge to Assess and Rehab Therapy Service to patients within the East Dorset Locality Area.
Key Responsibilities
- Deliver high-quality patient care and support to patients in their own homes and community settings.
- Work collaboratively with multidisciplinary teams to develop and implement effective care plans.
- Conduct comprehensive assessments and provide expert advice to patients, families, and carers.
- Develop and maintain strong relationships with local healthcare providers and community organizations.
- Contribute to the development and implementation of new services and initiatives to improve patient outcomes.
Requirements
- Registered Nurse with a valid UK nursing registration.
- Proven experience in community nursing or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic level of IT literacy and proficiency in using electronic patient records.
