About the Role

We are seeking a highly motivated and compassionate Social Care Professional to join our team at Oxfordshire County Council. As a key member of our Adult Services team, you will play a vital role in supporting individuals to maintain their independence and wellbeing in their communities.

Key Responsibilities
  • Empower individuals to identify their strengths and desired outcomes, promoting a strength-based approach to social care.
  • Utilize a person-centered approach to support individuals and carers, ensuring their voice is heard through advocacy where necessary.
  • Work collaboratively with Occupational Therapists, hospital teams, and safeguarding teams to provide comprehensive support to individuals.
