About the Role

We are seeking a highly skilled and dedicated Health Visitor to join our team at Essex Child and Family Wellbeing Services. As a Health Visitor, you will play a vital role in improving the health and wellbeing of children and families in our community.

Key Responsibilities
  • Deliver the Healthy Child Programme, assessing health needs, developing and implementing care plans, and evaluating their effectiveness.
  • Use the Family Health Needs Assessment tool to plan and evidence care, ensuring that each child receives tailored support.
  • Develop and maintain high-quality care plans, actively involving children, parents, and carers in the planning and implementation process.
  • Identify and assess health, growth, and development in children aged 0-5, using evidence-based practice to inform your work.
  • Use the Early Help Assessment Tool to identify unmet needs and develop targeted interventions.
  • Develop and maintain a high-quality standard for Specialist Public Health Nurse/Health Visiting practice, using evidence-based approaches to drive positive change.

Requirements
  • Specialist Community Public Health Nursing qualification (SCPHN)
  • Registered Nurse with current NMC registration
  • Experience in leading interventions to improve outcomes and safeguard children and families
  • A valid UK driving licence, with access to a car for work purposes
