Job Overview

The successful candidate will exhibit strong analytical skills and a compassionate approach, serving as an experienced nurse who, within their professional scope, will deliver care to patients from the initial assessment through to diagnosis, treatment, and ongoing evaluation. They will demonstrate proficient clinical judgment and provide expert care, including assessment and diagnostic capabilities, for patients within the primary care setting.

The individual will be dedicated to applying critical thinking in the clinical decision-making process. They will collaborate effectively with the primary care team to address patient needs, supporting the implementation of policies and procedures while offering nursing leadership as necessary.

The Practice Nurse will strive to deliver a high-quality, cost-effective service to the patients of the Primary Care Network (PCN), conducting clinical assessments, diagnosing conditions, and evaluating care within established protocols set by the Clinical Nurse Manager or Clinical Director.

A key objective of this role is to enhance patient health and wellbeing through improved identification and management of diseases.

The post holder will be required to demonstrate safe clinical decision-making and provide expert care, including assessment and diagnostic skills. They will prioritize and triage patient needs accordingly, making necessary referrals for investigations as appropriate.

Key Responsibilities

CLINICAL DUTIES:

- Assess and develop treatment plans for Chronic Disease Management for patients with relevant conditions.

- Conduct clinical evaluations and assess patient needs from both physiological and psychological perspectives, planning care accordingly.

- Diagnose, plan, implement, and evaluate interventions for patients with complex needs, proactively identifying and managing treatment plans for those at risk of developing long-term conditions.

- Support patients in adhering to prescribed treatments and provide information on medications, including potential side effects and interactions.

- Prioritize health issues and intervene appropriately in complex, urgent, or emergency situations, including initiating effective emergency care.

- Encourage patients to adopt health promotion strategies that foster healthy lifestyles and self-care principles.

- Manage health needs of women seeking family planning, cervical cytology, or sexual health consultations.

- Identify and refer patients with mental health needs in accordance with relevant guidelines.

- Participate in vaccination and immunization programs for both adults and children.

- Provide advice and administer vaccinations for patients traveling abroad, as appropriate.

- Deliver evidence-based care for patients with aural conditions.

- Address the needs of patients requiring opportunistic wound care.

Clinical Governance

- Ensure home visits are conducted safely, adhering to organizational policies.

- Maintain confidentiality of patient information and comply with information governance policies.

- Prescribe treatments in line with local and national guidelines.

- Accurately document all consultations and treatments in patient records.
