Care Coordination Specialist

Brackenberry Ltd is collaborating with a Local Authority to recruit a Care Co-ordinator for a 3-month contract, with the potential for extension based on client needs. We invite qualified candidates to submit their CV for consideration.
Compensation: £19.93 per hour
Key Responsibilities:
- Conduct initial assessments within the designated area of expertise.
- Oversee, evaluate, and update care plans in collaboration with users, caregivers, and relevant agencies.
- Ensure compliance with the council's Financial Regulations at all times.
- Utilize management information systems and technology for effective data recording, retrieval, and analysis as required by the department.
- Receive supervision from the direct line manager and be evaluated against established standards and objectives.
Required Qualifications:
- Proficiency in spoken English is essential for effective communication with the public and providing accurate advice.
- Capability to assess needs and develop suitable support plans, along with a solid understanding of the care management process.
- Ability to collaborate with service users, caregivers, and other professionals while prioritizing the service user in all activities.
- Experience in handling safeguarding cases and a thorough understanding of safeguarding protocols.
- Knowledge of departmental financial policies and controls, with a focus on delivering best value services and community placements.
- Exhibit key behaviors such as accountability, openness, respectfulness, active listening, and teamwork to achieve solutions.
Additional Information:
- Immediate availability or short notice is required.
- Right to work in the U.K. is mandatory.
- A basic DBS check is necessary for this role.
