Job Summary:

We are seeking an experienced and skilled Practice Nurse to join our team at St Luke's Medical Practice, a dynamic and forward-thinking primary care provider in NHS Scotland. As a key member of our healthcare team, you will be responsible for delivering high-quality patient-centered care to our patients.

Key Responsibilities:

  • Manage long-term (chronic) conditions, including Diabetes, Coronary Heart Disease, Hypertension, Atrial Fibrillation, Heart Failure, and Chronic Kidney Disease.
  • Work autonomously within your scope of practice, demonstrating a high level of clinical expertise and decision-making skills.
  • Collaborate with our multidisciplinary team, including GPs, district nurses, health visitors, and other healthcare professionals, to provide comprehensive patient care.
  • Participate in the development and implementation of clinical guidelines and protocols to improve patient outcomes.
  • Contribute to the education and training of junior staff members, including medical students and junior nurses.

Requirements:

  • Advanced practice nursing qualification and registration with the Nursing and Midwifery Council (NMC).
  • Proven experience in managing long-term conditions and working in a primary care setting.
  • Strong clinical skills and knowledge of relevant clinical guidelines and protocols.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work autonomously and as part of a team, with a high level of flexibility and adaptability.
