Job Title: Support Worker

We are seeking a compassionate and caring Support Worker to join our team, providing support and care to individuals in their homes and in the community.

About the Role

As a Support Worker, you will be responsible for providing person-centered care and support to individuals with varying needs, including rehabilitation, learning disabilities, and mental health.

You will work closely with our team to develop and implement personalized care plans, ensuring that each individual receives the support and care they need to live independently.

Key Responsibilities

  • Provide person-centered care and support to individuals with varying needs
  • Develop and implement personalized care plans
  • Work closely with the care team to ensure seamless delivery of care services
  • Support individuals with daily living tasks, including personal care and household chores
  • Accompany individuals on outings and activities, promoting social inclusion and community engagement

Requirements

To be successful in this role, you will need to be a caring and compassionate individual with excellent communication skills. Experience working with autism or non-verbal clients is desirable.
