About the Role:

An exciting opportunity exists for a dedicated and experienced Registered Nurse (RNLD/RMN/RGN) to serve as a Clinical Nurse Reviewer in the Northeast of England. This position is available during Non-Term Time only.

This community-oriented role provides a platform to enhance your assessment and decision-making capabilities in a supportive yet challenging environment.

You will establish and maintain strong relationships with health and social care professionals to promote effective multi-disciplinary collaboration and gain a comprehensive understanding of local NHS and independent sector services in clients' homes and care facilities.

The Clinical Nurse Reviewer will report to the Head of Commissioning and will be expected to:

  • Demonstrate a high level of motivation and a desire to acquire new skills;
  • Work autonomously on a daily basis;
  • Exhibit excellent verbal and written communication skills with meticulous attention to detail;
  • Possess strong people management abilities, adept at handling challenging situations and individuals with composure;
  • Engage comfortably with a diverse range of ages and individuals with various complex and clinical conditions;
  • Prescribe care based on individual health requirements;
  • Uphold a strong commitment to quality and patient/career experience;
  • Manage time effectively and thrive in a fast-paced environment;
  • Demonstrate proficiency in Microsoft Office and possess intermediate typing skills;
  • Adapt to changes at short notice;
  • Travel daily across the region and occasionally nationwide.

Main Responsibilities:

  • Collaborate within the commissioning team and as part of a broader multi-disciplinary team (MDT) both internally and externally.
  • Conduct comprehensive care needs assessments.
  • Facilitate multi-disciplinary assessments and resident reviews in accordance with established policies and procedures, working closely with Home Managers.
  • Manage cases as directed by the Head of Commissioning.
  • Act as Duty Clinician as necessary.
  • Ensure data systems are accurately maintained in line with operational policies.
  • Comply with the Data Protection Act 1998 and uphold confidentiality standards.
  • Engage in ongoing professional development and clinical supervision as per NMC guidelines.
  • Ensure adherence to all statutory and mandatory training requirements.
  • Maintain professional registration through revalidation.
  • Provide coverage for other Clinical Nurse Reviewers as needed.
  • Maintain professional and appropriate communication at all times.
  • Support the Continuing Healthcare department in achieving departmental goals and objectives.
  • Foster constructive relationships with a wide range of internal and external stakeholders.
  • Communicate relevant information, risks, issues, and dependencies, including briefings and reports.
  • Collaborate closely with all care providers involved in service delivery.
  • Coordinate and participate in relevant meetings, reporting attendance and providing information, advice, and support as requested.
  • Ensure that collected data is reported appropriately and monitored.
  • Provide accurate and legible documentation of information. Assist and support individuals and their representatives.
  • Enhance current management information systems to improve decision-making processes.
  • Conduct research and development as directed.
  • Carry out reviews.
  • Perform other delegated duties as required.
  • Ensure compliance with all statutory and mandatory training.
  • Fulfill any additional responsibilities as reasonably requested by the line manager.
