Care Coordinator Job Description

We are seeking a highly skilled and organized Care Coordinator to join our team at Home Instead. As a Care Coordinator, you will play a vital role in ensuring the smooth operation of our care services, working closely with clients, care professionals, and other teams to deliver exceptional care experiences.

Key Responsibilities:
  • Client Scheduling: Develop and maintain client schedules, ensuring that care professionals are matched to clients' needs and preferences.
  • Care Professional Management: Work with the recruitment team to ensure sufficient staffing levels, and collaborate with the care team to resource and schedule care packages.
  • Communication and Liaison: Build strong relationships with clients, care professionals, and other teams, ensuring effective communication and collaboration.
  • Scheduling System Management: Maintain accurate and up-to-date records in our electronic scheduling system, ensuring seamless care delivery.
  • Problem-Solving and Adaptability: Respond to schedule changes, last-minute cancellations, and other challenges, demonstrating flexibility and problem-solving skills.
Requirements:
  • Care Experience: Ability to provide care to clients, with a strong understanding of care needs and preferences.
  • Scheduling Experience: Proven experience in scheduling roles, preferably in home care or a similar environment.
  • IT Skills: Proficiency in Microsoft Office or Google Suite, with experience in CRM software and the ability to learn new technologies.
  • Communication and Interpersonal Skills: Excellent communication and interpersonal skills, with the ability to build rapport quickly and work effectively in a team.
  • Attention to Detail: Strong attention to detail, with the ability to multitask and work accurately under pressure.
