We are seeking a highly skilled and experienced Adult Mental Health Practitioner to join our Crisis Intervention & Home Treatment Team. As a key member of our team, you will be responsible for providing high-quality clinical services to children and young people with mental health needs at point of crisis.
Main Duties- Direct and oversee the day-to-day delivery of clinical services provided by the Crisis Intervention/Home Treatment service.
- Contribute to the clinical management of a team caseload of clients with complex needs, using CPA principles and evidence-based/client-centered principles to assess, plan, implement, and evaluate interventions.
- Support care coordination within the Tier 3 teams and attend relevant team meetings on behalf of the CIHTT, ensuring that issues/discussions are fed back to the wider team.

Person Specification- NMC Registration or other relevant professional registration body.
