About the Role

We are seeking an experienced Advanced Clinical Practitioner to join our team at Humber Primary Care. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the development of our primary care services.

Key Responsibilities
  • Provide expert clinical care to patients, utilizing autonomous clinical and prescribing skills, and a broad and in-depth theoretical knowledge base of primary care.
  • Work collaboratively with the general practice team to meet the needs of patients and support the delivery of policy and procedures.
  • Support the development and training of advanced clinical practice within primary care, promoting excellence in patient care and service delivery.
  • Contribute to the development of the service, working closely with the Practice Manager, Operations Lead, and GP Lead.
  • Collaborate with the Primary Care Matron to standardize policies and patient care, ensuring consistency and quality across our services.
Requirements
  • Current professional registration (HPC or NMC)
  • Independent Non-Medical Prescribing qualification
  • Education to Degree level
  • Qualification in Minor Illness/Minor Injuries
  • Advanced understanding of relevant clinical practice, standards, and protocols within identified clinical areas
Desirable Qualifications and Experience
  • Teaching qualification
  • Minor Illness / Injury
  • Leadership qualification
  • Paediatric Minor Illness / Ailments
  • Independent Non-Medical Prescriber for Adults and Children
  • Good working knowledge of relevant IT systems
