Advanced Nurse Practitioner

We are seeking an enthusiastic and motivated professional to join our team as an Advanced Nurse Practitioner. As a specialist resource, you will provide high-quality, patient-centered care to our patients, working collaboratively with clinicians, nursing teams, and other multidisciplinary teams.
Main Responsibilities- Provide holistic care to patients, assessing, diagnosing, and treating a range of conditions, including unscheduled minor conditions and acute illnesses.
- Work autonomously as a practitioner, making safe and effective clinical decisions, and referring or directing patients to other services as needed.
- Develop and implement care plans, promoting self-care and empowering patients to make informed choices about their treatment.
- Participate in and influence efforts to shape multi-disciplinary pathways, improving patient choice, quality of life, and self-management.
- Provide strong leadership to the team, including day-to-day leadership responsibilities, development, and creative leadership.
