As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our multi-disciplinary team.

Key Responsibilities:

  • Provide expert care to patients with chronic diseases, including management of conditions such as diabetes, hypertension, and asthma.
  • Conduct cervical screening and family planning services, ensuring a high level of patient satisfaction and safety.
  • Work collaboratively with colleagues to deliver comprehensive General Practice nursing duties, including health promotion, disease prevention, and patient education.
  • Support and communicate effectively with patients, colleagues, and other healthcare professionals to ensure seamless care delivery.
  • Contribute to the development and implementation of practice policies and procedures, ensuring alignment with NHS Scotland standards and guidelines.

Requirements:

  • Proven experience in chronic disease management, cervical screening, and family planning services.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a high-pressure environment.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Knowledge of NHS Scotland policies and procedures, with a commitment to upholding the highest standards of patient care and safety.

Preferred Qualifications:

  • Postgraduate qualification in a relevant field, such as chronic disease management or community nursing.
  • Experience working in a General Practice setting, with a strong understanding of the NHS Scotland framework.
