Job Summary

We are seeking a Practice Nurse to work approximately 30-36 hours per week at our purpose-built premises in Marshside, Southport. The successful candidate will be responsible for overseeing the provision of chronic disease identification, monitoring, and management, as well as delivering patient assessment and treatment in accordance with national and local guidelines.

Main Responsibilities

The Practice Nurse will be responsible for:

  • Reviewing and recommending changes to patient medications for chronic diseases where appropriate
  • Assessing the individual needs of patients and ensuring their healthcare is appropriate, evidence-based, and in accordance with the national GP contract and our Local Quality Contract (LQC)
  • Identifying, managing, and supporting patients at risk of developing long-term conditions through health promotion programs
  • Developing, implementing, and embedding an effective care management program for frail and housebound patients
  • Performing practical investigations, including phlebotomy, processing of patient specimens, cervical cytology screening, and electrocardiograms
  • Implementing and administering national vaccination programs for adults and children, and coordinating the provision of travel medication services
  • Maintaining accurate medical records in accordance with extant legislation and ensuring the use of appropriate clinical templates and SNOMED-CT codes
  • Maintaining the Practice Nurse and treatment rooms, ordering, stocking, and rotating items as required, and ensuring their general tidiness and cleanliness
  • Recognizing medical emergencies and supporting the team in their management, performing cardio-pulmonary resuscitation, and managing anaphylaxis in accordance with UK guidelines
  • Leading on Infection Prevention and Control (IPC) and working to the IPC policy, supporting the implementation of audits, training, and evidence for compliance to the Practice Manager
  • Ensuring adherence to the practice policies and procedures at all times, including safeguarding, data protection, and chaperone policies
  • Attending and participating in relevant practice/locality meetings when required, and supporting the Practice Manager and Partners with the implementation of any new clinical processes

Person Specification

The ideal candidate will have:

  • Experience working in a primary care environment
  • Ability to work autonomously
  • Knowledge of Infection Prevention and Control (IPC) measures
  • Clinical knowledge and skills, including phlebotomy, processing of patient specimens, electrocardiograms, patient group directions, good interpersonal and communication skills, accurate medical record keeping, effective time management, and competent use of MS Office software
  • Desirable skills include working as a practice nurse or community nurse, chronic disease management, childhood immunization and cervical smears, managing frail and housebound patients, and clinical knowledge and skills in chronic disease management, cervical cytology screening, vaccinations, national and local GP surgery performance indicators, awareness of national and local health priorities, problem-solving and analysis, clinical audit and risk management, and competent use of EMIS Web software

Other Requirements

The successful candidate will be required to work outside of core office hours for the delivery of flu clinics and be flexible to meet the needs of the practice.
