Job Summary

The Personal Support Worker provides personalized support and homemaking services to clients and families within the framework of VON Canada (Ontario) policies, procedures, and standards. The PSW collaborates with members of the healthcare team to meet the needs of clients and achieve an optimal level of independence and well-being.

Key Responsibilities
  • Conduct thorough assessments of clients' homes, personal situations, and care plans to identify areas for improvement and ensure their needs are met.
  • Contribute to the development of personalized care plans under the direction of the Personal Support Program Manager.
  • Implement changes to care plans as directed by the Manager.
  • Provide regular updates to supervisors and the Personal Support Program Manager on any changes in clients' needs.
  • Perform personal support tasks tailored to clients' needs.
  • Provide personal care as directed by the Personal Support Program Manager or immediate supervisor.
  • Offer simple bedside care, where required, under the guidance of a nurse.
  • Provide respite care to clients as needed.
  • Support clients' families by offering childcare services when required.
  • Organize daily activities, prioritizing personal support and other tasks.
  • Complete daily call checks and other required forms.
  • Assess clients' communication patterns and ability to relate.
  • Communicate effectively with healthcare team members regarding clients' needs.
  • Comply with Occupational Health and Safety legislation and VON Canada (Ontario) policies.
  • Function in accordance with VON Canada (Ontario)'s risk management policies.
Education, Designations, and Experience
  • High school diploma.
  • Secondary school education is preferred.
  • CCA Certification is required in Nova Scotia.
  • Must have a certificate in Home Support Worker, Personal Support Worker program, or Health Care Aide.
  • Must have a current Standard First Aid Certificate or CPR level C Certificate.
  • Some household management experience and experience working with elderly or ill individuals or children.
Skill Requirements
  • Strong communication skills, both verbally and written.
  • Able to communicate effectively with clients and families.
  • Good assessment skills.
  • Awareness of community resources.
  • Ability to work effectively in a multidisciplinary team or independently.
  • Able to take direction well.
  • Ability to adjust to working in different social and economic environments.
  • The capacity to adhere to strict confidentiality.
Working Conditions and Physical Demands

This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics.
