Job Summary

We are seeking a highly motivated and experienced Clinical Pharmacist to join our team at St. Andrew's House Medical Centre. As a Clinical Pharmacist, you will play a vital role in providing pharmaceutical support to our patients and working closely with our healthcare team to improve patient care and outcomes.

Main Responsibilities
  • Work with our Lead Medicine Management GP to develop and implement quality improvement projects and achieve PCN network DES goals.
  • Provide patient-facing services, including chronic disease management, repeat prescription management, and structured medication reviews.
  • Manage and review patients with polypharmacy, working within your clinical competencies to provide specialist pharmaceutical support.
  • Collaborate with our healthcare team to improve care and support safe and effective prescribing.
  • Contribute to quality improvement and audit initiatives, as well as managing aspects of the Quality and Outcomes Framework.

St. Andrew's House Medical Centre is a dynamic and patient-centered practice located in a large Victorian building. We are part of the Stalybridge, Mossley, and Dukinfield Primary Care Network and work closely with our community partners to provide high-quality care to our patients.

Key Requirements
  • Completed the 18-month CPPE Pathway or in the process of achieving it.
  • Mandatory registration with the General Pharmaceutical Council.
  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Clinical Diploma.
  • Previous working experience in General Practice.
  • Demonstrates and understanding of, and confirms to, relevant standards of practice.
  • Follows professional and organizational policies/procedures relating to performance management.
