About The Role

Support Worker

We are seeking a caring and patient individual to join our team as a Support Worker. As a Support Worker, you will play a vital role in assisting patients with their discharge from Daisy Hill Hospital in Newry.

Key Responsibilities:

  • Acting on referrals from Daisy Hill Hospital to collect patients who are ready for discharge.
  • Transporting patients home using a British Red Cross vehicle.
  • Ensuring patients are settled at home following their hospital discharge.

Requirements:

  • Thoughtful, caring, and patient with a natural way with vulnerable people.
  • Experience of working in a multi-disciplinary setting.
  • Ability to manage your own workload.
  • IT literacy.
