Position Overview
The Cardiac Rehabilitation Physiotherapist Specialist will engage in a pivotal role within the Cardiac Rehabilitation Service, contributing to the enhancement of patient care through a structured rehabilitation program.
Key Responsibilities
The successful candidate will:
- Manage a defined caseload within the Cardiac Rehabilitation Team, conducting thorough assessments and formulating tailored exercise prescriptions for patients with diverse cardiac conditions.
- Actively promote self-management strategies and connect patients with community resources upon completion of their rehabilitation program.
- Independently coordinate and oversee the patient flow through the phase 3 exercise segment of the Cardiac Rehabilitation initiative.
- Exhibit flexibility and adaptability throughout the workday to meet the evolving needs of the service.
