We are seeking a skilled Community Psychiatric Nurse to join our team on a flexible shift basis. As a Community Psychiatric Nurse, you will provide vital healthcare services within the community, making a significant impact on patients' lives while having the freedom to choose your working hours.
Key Responsibilities- Provide robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties.
- Identify appropriate care pathways and ensure the least restrictive option to meet the needs of the patient and ensure their well-being.
- Signpost to appropriate care providers (including primary and tertiary care).
- Raise concerns to senior staff for support and management.
- Complete documentation and planning care for patients.
- Present cases to Multi-Disciplinary Team for discussion around care pathway.
- Liaise with colleagues nursing, occupational therapy, medics, psychology, and admin.
- A minimum of 6 months paid experience as a Community Psychiatric Nurse in an UK hospital environment within the last 3 years.
- Comply with NMC Professional Registration and keep up to date with current nursing practices.
- Full UK driving licence and own vehicle.
