Job Summary:
We are seeking experienced and motivated care staff to join our team as Support Workers in a short break setting to provide care and support to individuals with a learning disability.
Key Responsibilities:
- Follow individual care plans and risk assessments to ensure the well-being and safety of service users.
- Contribute to the monitoring and assessment of service users' plans, reviews, and assessment of risks, ensuring compliance with organisational policies and procedures in line with Care Quality Commission Essential Standards of Quality and Safety.
- Work with the team to provide sensitive care and guidance to service users in areas of personal care, daily living, health, social integration, and emotional support.
- Develop and maintain effective relationships with service users, their families, and other stakeholders.
- Participate in the development and implementation of Person Centred Plans that meet individual needs.
- Work on a rota system, including days, afternoons, and nights over a 7-day week, including weekends and bank holidays.
Requirements:
- Good communication skills and the ability to use your own initiative and work unsupervised.
- Ability to write in daily notes within the service setting and use IT equipment.
- Flexibility and adaptability to meet the changing needs of service users.
