Key Responsibilities- Lead and coordinate all aspects of physical health needs of service users, while supporting colleagues and promoting good teamwork with the multidisciplinary team.
- Monitor observations and assessments to identify actions that need to be addressed and documented.
- Develop and support physical health interventions used in the clinical area, adhering to the NMC Code of Conduct and local, Trust, and national standards of practice.
- Engage actively in practice development, evaluation, audit activities relevant to physical healthcare and/or service areas to promote evidence-based practice.
- Work collaboratively with primary health, developing and maintaining links with GP practices, and other physical health departments.
- Monitor and maintain Physical Health Dashboards.
- A relevant nursing qualification and registration.
- Phlebotomy skills.
- Strong team-working skills.
- A high level of self-motivation and a flexible approach.
- A positive attitude and be naturally caring and compassionate.
- A commitment to high-quality care and to values and evidence-based practice.
- Understanding of the physical health care needs for individuals who are living with an acquired brain injury.
Our multidisciplinary team at Three Valleys Hospital provides care for individuals with complex mental health issues in a safe and supportive environment. We have a range of services across five wards, including a specialist dementia service, where you'll gain experience working with gender-specific distinct rehabilitation and step-down services.
What We Offer- Annual salary of £34,590.
- Monday to Friday 9am - 5pm working hours.
