About the Role

We are seeking a passionate and caring individual to join our team as a Care Team Leader at Care UK's Ayton House in West Sussex.

Key Responsibilities
  • Lead and supervise your shift to ensure the highest standard of care for residents.
  • Case manage residents and liaise with families, healthcare professionals, and other stakeholders to create and deliver effective care plans.
  • Administer and record medication, carrying out risk assessments as needed.
  • Work alongside care assistants to support residents with daily needs, promoting independence and activity.
  • Collaborate with the leadership team to develop services and provide a fun, inclusive, and professional approach at all times.
