Family Health Practitioner

We are seeking a highly skilled and dedicated Family Health Practitioner to join our Emotional Health and Resilience Service. As a key member of our team, you will work closely with children, young people, parents, and families to provide high-quality services that promote emotional well-being and resilience.
The successful candidate will have experience of working with and supporting parents and families, with excellent communication skills and the ability to handle sensitive data with confidentiality.
Main Duties of the JobAs a Family Health Practitioner, you will work as part of the children's community health commissioned service, contributing to the provision of high-quality services that meet the needs of children, young people, and their families. You will be pro-active in delivering the national and Public Health agendas, working in partnership with other agencies to meet agreed standards, objectives, and statutory requirements.
