Job Summary

We are seeking an experienced Registered Nurse to join our team as a Palliative Care Specialist. The successful candidate will be responsible for delivering high-quality care to patients in their own homes, working closely with our multidisciplinary team to ensure seamless care coordination.

Key Responsibilities
  • Clinical Practice
    • Provide professional telephone advice and support to patients, families, and healthcare professionals
    • Visit patients at home to assess their needs, develop care plans, and deliver direct care
    • Liaise with healthcare professionals and agencies to ensure effective care coordination
    • Contribute to clinical governance and quality assurance processes
  • Leadership and Management
    • Act as a role model and expert resource to the team, promoting high-quality care and best practices
    • Lead and manage a team of healthcare professionals to deliver high-quality care
    • Develop and implement care plans, ensuring patient-centered care and effective resource allocation
  • Education and Professional Development
    • Participate in the development and delivery of education programs for healthcare professionals
    • Contribute to the development of policies and procedures, ensuring compliance with regulatory requirements
    • Stay up-to-date with the latest research, best practices, and regulatory requirements in palliative care
Requirements
  • Registered Nurse with NMC registration
  • Post-registration qualification in community nursing, oncology, or palliative care
  • Experience in palliative care, preferably in a community setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
