Job Title: Community Diabetes Specialist Nurse

About the Role:

We are seeking an experienced and motivated Community Diabetes Specialist Nurse to join our dynamic and friendly Diabetes team for a 6-month fixed-term contract. As a key member of our team, you will play a vital role in delivering patient-centered care and support to individuals with diabetes.

Key Responsibilities:

  • Deliver high-quality patient care and education to individuals with diabetes, promoting self-management and optimal glycaemic control.
  • Collaborate with multidisciplinary teams to provide comprehensive care and support to patients with complex diabetes needs.
  • Develop and implement individualized care plans, taking into account patients' unique needs and circumstances.
  • Provide education and support to patients and their carers, empowering them to manage their condition effectively.
  • Contribute to the development and implementation of diabetes services, ensuring they are patient-centered and evidence-based.

Requirements:

  • NMC registration and specialist diabetes qualification.
