About the Role

We are seeking a skilled and compassionate Nurse Associate to join our Community Nursing Team based in Kent.

As a Nurse Associate, you will play a vital role in delivering high-quality patient care in the community, working closely with our team of healthcare professionals to provide personalized care to patients with complex and long-term conditions.

Key Responsibilities
  • Undertake holistic assessments of patients to plan care in collaboration with patients and their families.
  • Work as part of a team to deliver nursing support to patients in their homes, promoting independence and well-being.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure seamless care delivery.
  • Contribute to the development and implementation of care plans, ensuring they meet the individual needs of patients.
  • Participate in multidisciplinary team meetings to discuss patient care and share best practices.
Requirements
  • Registered with the NMC as a Nursing Associate registrant.
  • Knowledge of nursing theory and best practice at Nursing Associate level.
  • Experience of working as part of a team and contributing to the delivery of high-quality patient care.
  • Ability to communicate effectively with patients, families, and healthcare professionals.
  • Valid driving license and access to a car for work purposes.
