Job Title: Band 5 Student District Nurse

We are seeking a highly skilled and dedicated Band 5 Student District Nurse to join our team at Essex Partnership University NHS Foundation Trust. As a key member of our Dementia Intensive Support Service, you will play a vital role in providing high-quality care to patients with dementia and their families.

Job Summary:

The successful candidate will be responsible for providing rapid assessments for people living with dementia or suspected dementia who are in a crisis situation that requires home treatment to avoid hospital admission. You will also support the newly forming Functional Intensive Response and Support Team (FIRST), which provides assessment, support, and appropriate care planning to prevent unnecessary hospital admission and support early discharge when the individual is admitted.

Main Duties:
  • Provide high-quality, evidence-based, holistic care to patients in line with your professional code of conduct and Trust policies.
  • Assess, plan, implement, and evaluate packages of care based upon individual service user needs, under the guidance of the Advanced Nurse Practitioner.
  • Manage and coordinate multi-disciplinary meetings, including referral, review, business, and handover.
  • Undertake a range of clinical duties, such as providing practical support and guidance to service users.
Person Specification:
  • Registered Mental Health Nurse or Registered General Nurse.
  • Good standard of written and spoken English.
  • Good standard of education.
  • Current UK driving license and access to a car for work purposes.
Desirable:
  • Postgraduate qualification or commitment to work towards this non-medical prescribing qualification.
  • Experience of working in the Mental Health environment.
  • Experience of working with System One or Paris.
