About the Role:This is an exciting opportunity for a social worker to work in a statutory learning disabilities team, providing care management and safeguarding support to individuals with learning disabilities and autism.
Key Responsibilities:- Assess and review individuals in receipt of care and support
- Develop and implement care plans to meet individual needs
- Work collaboratively with partner agencies to provide a comprehensive service
- Conduct statutory annual reviews and undertake S.42 work, MCA's, and best interest assessments
To be considered for this role, you must have a social work qualification, full SWE registration, and over 3 years of post-qualified experience in an adults services setting.
