Job Summary
We are looking for a motivated and compassionate individual to coordinate activities and wellbeing initiatives for our residents. As a Care Coordinator, you will work closely with our team to plan and deliver engaging and entertaining activities that promote the physical and emotional wellbeing of our residents.
Key Responsibilities
- Develop and implement activity plans that cater to the diverse interests and needs of our residents
- Collaborate with our team to ensure seamless delivery of activities and services
- Build meaningful relationships with residents and their families
- Monitor and evaluate the effectiveness of activities and make recommendations for improvement
- Stay up-to-date with industry developments and best practices in care coordination
Requirements
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Highly organized and able to prioritize tasks effectively
- Passionate about delivering exceptional care and service
- Relevant qualifications or experience in care coordination or a related field
