Join Active Care Group as a Care Manager

Active Care Group is a leading provider of high-quality complex care for individuals in their own homes. We are seeking an experienced Care Manager to join our team in Birmingham and surrounding areas.

About the Role

As a Care Manager, you will be responsible for delivering exceptional care to our clients, ensuring their needs are met and their independence is maintained. You will work closely with our support workers, clinicians, and management team to provide a safe, reliable, and outcome-based care service.

Key Responsibilities
  • Manage care packages in your area, ensuring high-quality care is delivered to clients
  • Produce assessment reports for care packages, highlighting client needs and preferences
  • Support with recruitment and training for support workers, ensuring they have the skills and knowledge to deliver excellent care
  • Work collaboratively with the team to ensure client-focused care is delivered at all times
Requirements
  • NVQ 4 or above in Health and Social Care
  • Full clean driving licence with access to own car
  • Passion for delivering high-quality client-focused care
  • Experience in care at Team Leader level or above
