**About the Role**

We are seeking an experienced Occupational Therapist to join our Short Term Assessment and Reablement (START) Team on a 3-year fixed-term contract. As an Occupational Therapist, you will play a crucial role in empowering individuals to live independently and achieve their goals.

**Key Responsibilities:**

  • Screen and prioritize Reablement occupational therapy referrals to ensure timely and effective support for individuals.
  • Work collaboratively with individuals to develop personalized plans to regain independence and achieve their goals.
  • Provide recommendations for equipment and adaptations to support individuals in their daily lives.
  • Collaborate with healthcare teams and carers to ensure seamless care coordination and effective communication.

**Requirements:**

  • Degree in Occupational Therapy, registered with the Health and Care Professions Council (HCPC).
  • Minimum 2 years of experience in Occupational Therapy, with a strong understanding of relevant legislation in social care.
  • Experience in working within a multidisciplinary team, with excellent communication and interpersonal skills.
