Job Summary

We are seeking an experienced social worker to join our dynamic Adult Community North team. As a crucial member of our team, you will collaborate with individuals, families, and caregivers to provide support and intervention for adults with physical disabilities, chronic conditions, autism, and sensory impairments.

Main Duties

As a social worker at this level, you will be responsible for conducting Care Act 2014 assessments, evaluating needs, and managing both short-term and long-term cases. You will also be involved in advancing safeguarding inquiries, handling Court of Protection matters, and maintaining close collaboration with healthcare and multidisciplinary professionals.

Key Responsibilities
  • Practice in accordance with and uphold the Standards of Professional Practice for social workers as defined by the Health and Care Professions Council.
  • Practice in accordance with the Professional Capabilities Framework for Social Workers at the level of Experienced Social Worker.
  • Manage complex caseloads, offer expert opinion within the organisation and to others, and chair a range of meetings.
  • Model good practice, set expectations for others, and take responsibility and be accountable for the practice of others.
Person Specification

We are looking for a qualified social worker with evidence of post-qualifying learning and development. You will need to be registered with Social Work England and have a proven track record of strengths-based social work with adults, including the safeguarding of adults. You will also need to have extensive knowledge of health and social care and other relevant legislation, policies, and guidance.
