We are seeking an experienced pharmacist to join our team as an Advanced Pharmacist in the Anticoagulant and Thrombosis Service. This is an exciting opportunity to work alongside our Thrombosis and Thromboprophylaxis Committee and contribute to improving patient care.

About the Role

This full-time position involves providing advanced level clinical pharmacy services to patients with complex anticoagulation needs. You will work closely with healthcare professionals to optimise patient care and develop a highly specialist knowledge of thrombosis and thromboprophylaxis.

Key Responsibilities
  • Act as a specialist pharmacist providing advanced level clinical pharmacy services to patients with complex anticoagulation needs.
  • Communicate complex drug information to prescribers, clinicians, patients, and relatives.
  • Develop a highly specialist knowledge of thrombosis and thromboprophylaxis to promote evidence-based medicine.
  • Prescribe and deprescribe medicines for agreed patients within your capacity as an Independent Prescriber.
  • Participate in multidisciplinary ward rounds, Thrombosis & Thromboprophylaxis specialist team MDT meetings, and clinics for Thrombosis & Thromboprophylaxis patients.
Requirements
  • Masters Degree in Pharmacy.
  • Qualified pharmacist registered with the General Pharmaceutical Council.
  • Independent prescriber.
  • Significant demonstrable experience as a clinical pharmacist.
