Position Overview

We are pleased to announce an opportunity for a Band 6 Physiotherapist within the Community Division of our esteemed organization. We are seeking a qualified, motivated, and proactive individual to join our Community Therapy Team.

The successful candidate will collaborate within a multi-disciplinary team to manage a specific caseload, providing essential care to patients in their homes and the wider community. This role includes offering senior support and guidance to fellow team members, participating in triage processes, and assisting team leaders in service delivery.

Key Responsibilities

  • Conduct comprehensive functional assessments for individuals in both acute and community environments.
  • Occasionally work in residential care settings to identify obstacles preventing patients from returning to their usual living arrangements.
  • Design and implement effective therapeutic interventions, collaborating closely with other community services to address identified functional challenges while managing associated risks.
  • Serve as a knowledgeable resource for managing a diverse range of conditions, providing advice to patients, their families, and other healthcare professionals.
  • Assist the Team Leader in the planning, coordination, and evaluation of daily service operations.
  • Actively participate as a member of the multi-disciplinary team, contributing to integrated health and social care initiatives.
  • Manage a defined caseload using evidence-based and client-centered approaches to assess, plan, implement, and evaluate interventions.
  • Supervise students during their practical placements and provide leadership and clinical oversight to junior staff as directed.
  • Support team leaders in developing and delivering in-service training for multidisciplinary colleagues.
  • Engage in service development activities alongside senior colleagues, including data collection, audits, and service reviews.

Qualifications and Experience

  • A Diploma or Degree in Physiotherapy.

  • Proficiency in supervising students and staff from various professions.
