Position Overview
The Community Mental Health Team is in search of a dedicated, proactive, and well-organized Mental Health Practitioner to join our established integrated, multi-disciplinary team. This role focuses on supporting individuals facing severe and persistent mental health challenges. The successful candidate will be responsible for the care coordination and case management of a specific caseload of clients with complex and demanding needs, as outlined in the Mental Health (Wales) Measure.
Main Responsibilities
- Oversee the care coordination of a designated group of clients experiencing severe and enduring mental health issues.
- Ensure compliance with Care and Treatment Plans, meeting standards as per the Mental Health Measure (Wales), and serve as a resource for General Practitioners and other interfacing services, providing guidance, support, and referrals to suitable agencies.
- Develop and implement both standard and complex outcome-focused care and treatment plans.
- Conduct comprehensive risk assessments, documenting actual and potential risks.
Language Proficiency
Proficiency in Welsh is preferred for this position; applications from both Welsh and English speakers are encouraged.
