Job Summary

We are seeking a highly motivated and organized individual to join our team as a Health Check Coordinator. This is a new role within our practice as part of a pilot to improve health needs across our patient demographic.

Main Responsibilities

The successful candidate will be responsible for identifying patients who have not had a blood pressure check in the last 5 years and targeting this group specifically. They will then invite these patients in for a health check, which will involve carrying out height, weight, and blood pressure checks.

Following the patient journey from invite to health check to further signposting is a crucial part of the job. The successful post holder will undergo a phlebotomy 2-day course to be able to carry out this work safely and effectively.

Key Duties

  • Identify patients who have not made contact with the practice in the last 5 years.
  • Carry out NHS health checks with patients, including height, weight, and blood pressure checks.
  • Signpost patients to health coaches for lifestyle advice, including diet and exercise.
  • Offer blood tests to check for chronic conditions, ideally with phlebotomy training.

About the Role

This is a one-year post that will engage and complete NHS health checks with hard-to-reach patients. The post holder will be supported by our clinical team to follow up with patients and ensure that they receive the necessary care and support.

Person Specification

We are looking for a candidate with the following qualifications and experience:

  • Desirable: Qualified in phlebotomy.
  • Desirable: Experience in general practice.
