The successful candidate will be a skilled Patient Care Support Specialist responsible for assisting in the management of specific patient groups with various health conditions. Duties will include:

- Conducting patient assessments and home visits as required.
- Performing essential health checks, including blood pressure monitoring, temperature readings, and other vital sign assessments.

This position is offered on a contractual basis, with potential for extension based on service needs. Travel allowances will be provided for home visit duties.

Qualifications and Skills
